As Legacy’s Senior Vice President of Care Management, Latonia Walker is responsible for overseeing Legacy’s Care Management Department which includes the following areas: Care Management, Health Navigation, Nutrition & Fitness, Patient Education, Prenatal Navigation, Social Services and Social Determinants of Health (SDOH).

The Care Management Department is responsible for providing valuable services and support to assist the patient achieve improved health outcomes. Examples of these services include outreach after Emergency Department or Hospital discharges to assess & address unmet needs, facilitate follow-up visits with the patient’s primary care provider and provide care management support for patients with chronic or complex conditions. Providing patient education, nutrition services, health screenings, point of care testing, social services and linkage to care are also key functions of the Care Management Department.

A Registered Nurse for over 25 years, Walker earned a Bachelor of Science in Management from the University of Phoenix and a Master of Business Administration from Texas Woman’s University.

She is a native Houstonian who enjoys traveling, gardening and spending time with family.
